Make sure you choose the form that corresponds to your level of study (Master's or Doctoral)
Completion of the final master's (BIP6024) and final doctoral (BIP7001) seminar courses is conditional upon prior participation by students in a minimum of six (6) seminar or conference activities. Each student must print the certification form and bring a copy to these activities, so that it can be completed and signed by the person responsible for it. Then, he must send the 6 completed forms, before registering for the BIP6024 or BIP7001 courses, to the following address: dir.cycles.sup.bcm@uqtr.ca
This report is called Rapport d'évaluation trimestrielle. It is completed at the end of the term, by the student and his research director. It is an excellent tool for communication, planning, and adjustment. The student and his supervisor can record the results of the work completed during the term that is ending, as well as the work plan for the following term. They can also comment on the progress of the research project and the development of the student's skills. The research supervisor indicates on the form if a Satisfactory or Unsatisfactory rating is given for the term that is ending. If a research supervisor gives an Unsatisfactory rating for two separate terms since the beginning of the student's studies, he/she can assess whether or not he/she will continue the supervisory relationship with this student.
The report, completed and signed by the student and the supervisor, is sent to the following address: dir.cycles.sup.bcm@uqtr.ca. The program director reviews the reports and may, if necessary, act as a neutral third party to facilitate any adjustments required.
